Prairie Rehabilitation provides therapy to patients at multiple clinic locations across South Dakota and Minnesota. Treating patients in an outpatient facility is often the best option for quality patient care, but for some patient’s receiving therapy in their home is the only option.

Outpatient therapy in your home

Prairie Rehab AT HOME program serves patients when travel, fatigue, or other barriers prevent attendance at traditional outpatient clinic-based therapy programs.

In home therapy allows Prairie Rehab’s therapists to customize a rehabilitation program around the individual’s needs and interests, while also helping to maintain a safe and independent lifestyle. Our in-home outpatient therapy services are designed to improve both the patients’ and their families’ capacity to be successful in every aspect of their lives.

The program is designed for the general community. Patients may live in their own home, senior housing, or assisted living. This program is not intended for patients who are truly homebound who require additional skilled services as defined by Medicare.

Prairie Rehab AT HOME Candidates

You are a candidate for outpatient therapy in your home if you…

  • Do not qualify as homebound but have mobility problems that prevent you from going to an outpatient clinic setting.
  • Find traveling to an outpatient clinic difficult due to physical or emotional constraints
  • Have difficulty or inability securing transportation to an outpatient clinic
  • Desire to remain in your home for as long as possible
  • Require a high emphasis placed on improving ability to perform daily living skills within your home

Homebound status– Patients that have designated homebound status as certified by their physician and require skilled nursing care are required to receive therapy services through a certified Home Health agency and are not candidates for receiving in home outpatient services.

All of our therapy disciplines are available as part of the AT HOME program. Physical Therapy provides treatment and instructions on home safety, balance, orthopedic and neurological rehabilitation, with a focus on returning to independence.  Occupational Therapy helps patients restore or maintain their ability to perform tasks used in daily living, often through developing ways to modify or adapt activities. They assess and treat patients in order to increase their self-help and homemaking skills. Speech Therapy involves corrective treatment of physical and/or cognitive disorders that cause difficulty with communication and/or swallowing. They also teach patients and family members safe and appropriate swallowing techniques, all of which enable the patient to maintain independence and safety. Services are available based on your location. Speech therapy services are only available at select locations.

Is Prairie Rehab AT HOME Covered By Insurance?

For most senior citizen patients, the rehabilitation services are covered under Medicare Part B (not part of the Home Health Agency Prospective Payment System) with only a small co-payment by co-insurance. For non-seniors insurance typically covers at home therapy as they would if patient was seen in one of our outpatient clinics. We always verify insurance coverage prior to initiating therapy services.

Getting Started

Once you identify yourself or a family member who has a need for rehabilitation services but is not qualified or appropriate for home health or clinic based therapy, simply call us or fax it to us with a referral. A doctor referral is helpful but not required. We will be happy to contact the patient’s physician for the referral to therapy.  When we have the referral and information, we will schedule your first appointment.
